If you wish, you can fill out some of these forms at home and bring them with you to the appointment. Just <a rel=\"noreferrer noopener\" href=\"https:\/\/arizonahearing.com\/patient-forms\/\" data-type=\"page\" data-id=\"24\" target=\"_blank\">click here<\/a>, download the forms, and print them. This will save you some time. Please also bring the following applicable information with you:<\/p>\n\n\n\n<ul class=\"wp-block-list\"><li>Insurance card<\/li><li>Referral for your hearing evaluation from your primary care physician (if required by your insurance provider)<\/li><li>Your payment\/co-payment<\/li><li>A list of current prescriptions and\/or over-the-counter medications you are taking, including dose and frequency<\/li><\/ul>\n\n\n\n<p>After the forms are filled out, we will sit down and talk about your hearing concerns, complete a comprehensive audiological evaluation then talk about possible solutions.
